Monteggia’s fracture

Fracture of the ulna just below the elbow, with dislocation of the radius from the elbow joint.These injuries are relatively uncommon, accounting for fewer than 5% of all forearm fractures. The mechanism of injury is most often a fall on an outstretched hand.

The ulna fracture is usually clinically and radiographically apparent. The keys to successful diagnosis of a Monteggia fracture are clinical suspicion and radiographs of the entire forearm and elbow. 

The Bado classification system describes four types:

Type 1  Apex anterior angulation and anterior radial head dislocation.   

Type 2  Apex posterior angulation with posterior radial head dislocation.  

Type 3  Lateral radial head dislocation.  

Type 4  Fractures of both radius and ulna with radial head dislocation.

Giovanni Battista Monteggia at the age of 17 became an apprentice in surgery at the Great Hospital in Milano and from 1790 was surgical assistant and prosector of anatomy from 1791 prison physician, from 1795 professor of anatomy and surgery and, eventually, surgeon (Wundarzt) at the mentioned hospital. His work on surgery was highly praised by Antonio Scarpa (1752-1832). He acquired syphilis by cutting himself at autopsy.
